LIST OF DOCUMENTS TO BE ENCLOSED WITH THE REGISTRATION FORM

⇒ Recent coloured passport size photograph of the Child, Mother and Father duly pasted on the form in the space provided.

⇒ Self-Attested photocopy of Date of Birth Certificate of the child.

⇒ Affidavit for the girl child.

Any one of the following documents mentioned (as address proof):

⇒ Self-Attested copy of Ration Card/ Smart Card issued in the name of parents (Mother/Father having name of the child).

⇒ Self-Attested copy of Electricity/Water Bill/MTNL/ Passport in the name of any of the parent).

⇒ Self-Attested copy of Aadhar Card/UID of the child & his/her Parents.

⇒ Domicile Certificate of the child or of his/her Parents.
